To enable TelehealthMD.ca to provide our practitioner referral service, the site must collect personal information from its
users, and is committed to maintaining the privacy of this information in accordance with all applicable Federal, Provincial,
and Territorial laws and regulations. This Privacy Policy (hereinafter the “Policy”) is addressed to the users (hereinafter
the “User”) of services provided by TelehealthMD, a company incorporated according to the laws of Saskatchewan,
Canada, and its parent companies, holdings, affiliates, and subsidiaries (hereinafter, jointly, “TelehealthMD”), and will be
applicable and binding at all times while using the website.
I. Definitions
Cookies Data files that are stored in the hard drive of a user’s computer equipment or electronic
communications device when surfing a specific internet site, which allow the tracking of
status information between said site and the user’s browser. Status information may
reveal session identification, authentication, or user preferences, as well as any data
stored by the corresponding browser to the website.
Health Professional Type of user that is a professional who uses the Website, being a doctor, odontologist,
psychologist, psychiatrist, physiotherapist, nurse, nutritionist, therapist, or other person
not previously mentioned, that undertakes activities in the health services field and
provides health, medical, or paramedical services.
Owner Individual to whom the Personal Data belongs.
Patient Type of user who accesses the Platform to seek medical or mental health treatment or
advice from Health Professionals.
Personal Data Any information regarding an identified or identifiable individual.
Platform The software, applications, website, and content composing TelehealthMD.ca.
Sensitive Personal Data Personal Data that affects the most intimate sphere of its owner, or which undue use
may cause discrimination or a great risk for him/her/them. In particular, data considered
as sensitive is that which may reveal aspects such as racial or ethnic origin, present and
future health status, genetic information, religious, philosophical and moral beliefs,
union affiliation, political opinions, or sexual preference.
Third Party Individual or legal entity, national or foreign, different from the user or TelehealthMD.
II. Certain Interpretation Rules
In this Policy, TelehealthMD may be referred to, indistinctly, as the “Company” or “us”; additionally, the phrases “our” or
“ours” will be understood to refer to TelehealthMD.
This Policy is addressed to the Users of the Platform and will be applicable and binding at all times while using the website.
All persons who are registered, access, or use the Platform, by any device or computer, will be referred to as “Users”,
regardless of the access being via the mobile app or the website.
In this Policy, the plural includes the singular and the singular the plural.
The words “includes” or “including” shall be understood as “including, but not limited to”.
References to any Person or entity, that for purposes of this Policy means an individual, a company, a partnership,
association, trust, or other entity or organization, includes its successors and assignees.
III. Information collected
The Owner shall provide certain information, which is considered indispensable to be able to provide the service, at the
time of creating his/her/their account and is responsible to update the information as needed.

  3. Country of residence;
  4. Province of residence;
  5. City of residence;
  6. Phone number.
    From Health Professionals:
  7. Full legal name;
  8. Email address;
  9. Phone number;
  10. Licence and/or professional identification number;
  11. Name and address of practice;
  12. Practice website URL;
  13. Years of experience;
  14. Specialty or specialties.
    Patient additional optional information could be, but may not be limited to:
  15. Profile photo;
  16. Ailments;
  17. Clinical history;
  18. Age;
  19. Weight;
  20. Height;
  21. Date of birth.
    Additional information collected from Health Professionals could be, but may not be limited to:
  22. Profile photo;
  23. Academic background;
  24. Social media links
    a. Facebook;
    b. LinkedIn;
    c. Twitter;
    d. Instagram;
    e. YouTube; etc.
  25. Business hours.
    IV. Purposes for Personal Information Collection
    Personal Data collected shall be used for:
  26. Putting Users in contact via the Platform, specifically Patients with Health Professionals.
  27. Communication purposes.
  28. Identification purposes.
    V. Information sharing
    We do not disclose any personal information about our customers, past or present, to any entity except as described
    below and otherwise permitted or required by applicable law. We may disclose some of the information we collect, as
    described above, to the Health Professionals that provide medical services, advice, and information through
    TelehealthMD’s practitioner referral service, to assist them in providing patients with the medical service, advice, and
    information that has been requested or authorized.
    The personal information that you provide to the physicians and practitioners participating in TelehealthMD’s practitioner
    referral service is governed by doctor-patient confidentiality and the rules and regulations of the governing body of the
    Province or Territory of Canada where the practitioner is licensed to practise medicine (in most Provinces and Territories
    the “College of Physicians and Surgeons”). None of the information that you provide to the physicians and practitioners
    participating in TelehealthMD’s practitioner referral service is provided to TelehealthMD, and TelehealthMD is not a party
    to the doctor-patient relationship that exists between you and the physicians and practitioners providing you with medical
    services, advice, and information.
    In no event do we disclose your personal information to companies that will use that information to contact you about
    their own products or services.
    VI. Our Security Procedures
    We restrict access to personal information to those persons who need such information to provide products or services
    to you. We maintain physical, electronic, and procedural safeguards that comply with all Federal, Provincial, and Territorial
    laws and regulations to protect your personal information. We keep personal information only for as long as is necessary
    for our business purposes and will dispose of all personal information in appropriate ways according to applicable law and
    TelehealthMD’s own practices.
    VII. Changes to Privacy Policy
    TelehealthMD may revise and update this Privacy Policy at any time and without notice to you. You are cautioned and
    encouraged to review the Privacy Policy posted on this website periodically. Your continued use of the website will
    constitute a deemed acceptance of any changes.
    VIII. Cookies Policy
    TelehealthMD uses several technologies to improve the efficiency of its website, including the browsing experience.
    Among these technologies is the use of third-party cookies. Cookies are small amounts of information stored in the
    browser used by each user so the server may remember certain information that may be used later on.
    This information is used to identify you and to store your personal preferences to provide you a better browsing

    IX. Miscellaneous
    This Privacy Policy shall be governed by the laws of the Province of Saskatchewan and the Federal laws of Canada
    applicable therein. Any disputes shall be governed by the Courts of the Province of Saskatchewan without reference to
    conflict of laws principles.
    By using TelehealthMD’s website and its practitioner referral service, you acknowledge that you have read this Privacy
    Policy and agree to be bound by it.
